Harding Medical Institute ("HMI," "we," "us") respects your privacy. This policy explains what information we collect through hardingmedicalinstitute.com (the "Site"), how we use it, and the choices you have. Health information you share with us as a patient is additionally protected under our Notice of Privacy Practices.
Information we collect
- Information you give us. When you request a consultation, download a guide, refer a friend, take our hormone symptom check, or contact us, we collect what you enter — typically your name, email address, phone number, the service you're interested in, your notes to our team, and, for the symptom check, your answers and score.
- Information collected automatically. Like most websites, we use cookies and similar technologies (Google Analytics and HubSpot) to understand how visitors use the Site: pages viewed, links and buttons clicked, approximate location, device and browser type, and the site that referred you. This data is aggregated and does not identify you by name unless you also submit a form.
How we use information
- To respond to your requests, schedule consultations, and follow up on your inquiry.
- To send you the guide or information you asked for, and, if you opt in, occasional health education and practice updates. You can unsubscribe at any time using the link in any email.
- To understand how the Site is used and improve it.
- To protect the Site and our practice from fraud, spam, and misuse.
How we share information
We do not sell your personal information. We share it only with service providers who help us operate the Site and our practice (for example, HubSpot for our contact database and email, Google Analytics for site analytics, and our website hosting provider), and only for the purposes described here. These providers are contractually required to protect your information. We may also disclose information when required by law.
Information you give us about someone else
If you refer a friend or family member, you give us their name and contact details. Only do so if they have agreed to hear from us. We use that information solely to contact them about the practice, and they may ask us to delete it at any time.
Website forms are not a medical record
Our website forms and email are convenient ways to reach us, but they are not a secure channel for detailed health information. Please don't include diagnoses, medication lists, or test results in a web form. Once you become a patient, your health information is handled under our Notice of Privacy Practices and HIPAA.
Online screening tools
Self-assessment tools on the Site, such as the hormone symptom check, are provided for informational and educational purposes only. They do not provide medical advice or a diagnosis, and using them does not create a provider–patient relationship with Harding Medical Institute. If you have urgent or severe symptoms, contact a healthcare provider or call 911.
Cookies and analytics
You can control cookies through your browser settings. Blocking cookies may affect some Site features. To opt out of Google Analytics, you may install the Google Analytics opt-out browser add-on.
Text messages and calls
By providing a mobile number on one of our forms, you agree that Harding Medical Institute may call or send you text messages about your inquiry, appointments, and your results. These are conversational messages from our team, not automated marketing texts. Message and data rates may apply; message frequency varies. Reply STOP to any text to opt out, or HELP for help. Consent to texts is not a condition of receiving care.
Your choices and rights
You may ask us to correct or delete the personal information we hold about you, or to stop contacting you, by emailing info@hardingmedicalinstitute.com or calling (407) 210-2101. Requests involving your medical record are handled under our Notice of Privacy Practices.
How long we keep information
We keep website inquiry information for as long as needed to respond to you and to maintain our records, typically no longer than three years after your last contact with us unless you become a patient, in which case medical-record retention rules apply. You can ask us to delete your inquiry information sooner.
Do Not Track and third-party analytics
Our Site does not currently respond to browser "Do Not Track" signals. Google Analytics and HubSpot may set cookies that persist across visits; both providers' own privacy policies govern how they handle data on their side. We have configured analytics not to collect health answers entered into our screening tools as analytics events; only the fact that the tool was started or completed is recorded.
Children
The Site is not directed to children under 13, and we do not knowingly collect personal information from them.
Security
We use reasonable administrative, technical, and physical safeguards to protect your information. No method of transmission over the Internet is completely secure, so we cannot guarantee absolute security.
Changes to this policy
We may update this policy from time to time. The "last updated" date at the top shows when it last changed.
